Palliative Care

NHS Greater Glasgow and Clyde and Macmillan Cancer Support have developed a palliative care resources folder for community pharmacy. Gluten Free Food Service - Formulary

Community pharmacists may need to provide patients who have registered for the Gluten Free Food service with a copy of the local formulary for them to choose which items to order.

You can access the NHS GGC GFF formulary from the Greater Glasgow and Clyde Medicines website, where you can retain for use in the pharmacy or print off for patients.

Chronic Medication Service: Toolkit for Community Pharmacy

NHS Greater Glasgow and Clyde are delighted to publish the updated CMS toolkit for community pharmacy. A similar resource will be published shortly for general practice which will help to ensure similar processes and standardisation of advice with regards to the service.

This resource has been developed to help support community pharmacists with the implementation of both components of CMS: care planning and serial scripts. This includes documentation to help with processes, SOPs and generic care plans. In addition, PMR system specific training materials have also been developed but due to the sensitive nature of some of this part of the resource, we cannot publish on the website. Copies are available by request from CPDT.

Folders containing this toolkit are now available in each community pharmacy within NHSGG&C; a small number of spare copies are also available from CPDT on request but the aim is for locums, relief managers or field based managers to access the toolkit resources via the website functions.

Pharmacy Care Record (PCR) User Creation Request Form

As agreed by the Scottish Government (SG) and Community Pharmacy Scotland (CPS) a "web-based" centralised application, known as the Pharmacy Care Record (PCR), has been designed and implemented to enable pharmacies to create and maintain pharmacy care records for their patients.

With the introduction of the Chronic Medication Service (CMS) in April 2010, all pharmacies in Scotland must have the ability to maintain a pharmaceutical care plan (PCP) for their CMS patients and this plan will form part of the patient's pharmacy care record. The PCR and CMS PCP must be available at the launch of CMS.

Each pharmacy's set of pharmacy care records will be held in a secure central repository. Maintaining the confidentiality and integrity of this information is of paramount importance.

The PCR application must be accessible in all 1,200+ community pharmacies in Scotland but initially, user access to the PCR repository will be limited to authenticated pharmacists (PCR Users). As the PCR holds and maintains confidential patient records access must be strictly managed and controlled through agreed protocols.

All users that have access to the PCR for any purpose shall be identified with a unique username validated by a password.

In order to have a user account created for the PCR each pharmacist must complete the attached form and return to the Health Board.  A process will then be followed to create the user account and provide a password securely to each user.  Please ensure that all pharmacists working within your pharmacy complete the form, including locums.

Note: It is imperative that locums, who are not based in any one community pharmacy, provide an alternative address that can be used for communication of the password. Failure to provide an address may result in a delay to this process.
