Chronic Medication Service (CMS) factsheet

NHS Chronic Medication Service (CMS) factsheet. This is available in advance of community pharmacies receiving their supply of CMS patient leaflets which will be sent directly in due course. It covers the basic principles of the service and can be used to explain to patients what they can expect to receive as a result of registering for CMS. It should be printed and given to any patient that a community pharmacist is thinking about asking to register for the service. Due to the requirement for each community pharmacy to register no more than 50 patients during the initial implementation phase - from May to December - community pharmacists should not have CMS leaflets out on display.
