This functionality is a key part of the delivery of Medicines Care &amp; Review (MCR) and allows users to record care provided at the patient\u2019s registered Community Pharmacy (CP).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Key capabilities of the Pharmacy Care Record<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>search for and add patient (incl. CHI lookup)<\/li>\n\n\n\n<li>pharmaceutical care plan (patient profile, gluten-free diet, care issues and outcomes, Stage 1 and 2 medication review, and adding medication\/importing from PMR)<\/li>\n\n\n\n<li>high risk medicine (including care risk assessments for high-risk medicines)<\/li>\n\n\n\n<li>new medicine (including new medicines intervention support tool)<\/li>\n\n\n\n<li>support tools (gluten-free foods annual health check, smoking cessation)<\/li>\n\n\n\n<li>Situation, Background, Assessment &amp; Recommendation (SBAR) communication methods<\/li>\n\n\n\n<li><strong>pharmacy<\/strong>&nbsp;home (summary totals with links to detailed information)<\/li>\n\n\n\n<li>Reports.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Patient Care Record<\/strong>&nbsp;(PCR) Username and Passwords<\/h2>\n\n\n\n<p>All registered Pharmacists and Pharmacy Technicians in Scotland, working in community pharmacy require a PCR User ID and password to access Patient Care Records.
