Please sign up and return the authorisation to <a href=\"mailto:fife.pgd@nhs.scot\">fife.pgd@nhs.scot<\/a>. The authorisation&nbsp;forms can be found separately for completion&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/%7BlocalLink:umb:\/\/document\/05cc22ec1f984bc7915bbef98f82f7d2%7D\">here<\/a>&nbsp;<\/p>\n\n\n\n<p>To accompany the Pharmacy First PGDs there are nationally agreed assessment forms and GP notification forms editable versions&nbsp;are provided&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/%7BlocalLink:umb:\/\/document\/ae509ee49a1c4239a4a73fdac171bd43%7D\">here<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Other Useful Forms<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3352\/2020-05-11-nhs-pharmacy-first-privacy-note-final.docx\">Privacy Notice<\/a> for display in the pharmacy.<\/li>\n\n\n\n<li><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3338\/pfs-consultation-form-template-july-2020.pdf\">Consultation Forms<\/a> which may be used for recording consultations in advance of inputting into the PMR.<\/li>\n\n\n\n<li><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3529\/nhs-fife-pharmacyfirstreferral.docx\">Pharmacy Non Urgent Pharmacy First Referral Form<\/a> which may be used to provide the patient with information to pass on to&nbsp;the partner service you are referring them to.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Training<\/h2>\n\n\n\n<p>Please see below a message from the Scottish Public&nbsp;Health Network regarding a new Turas module available for completion:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>&#8220;As part of Our Health Matters, a series of projects developed to tackle the health inequalities experienced by Scotland\u2019s Gypsy\/Traveller community, we have developed a new Turas Learn module called Communication Matters.\u00a0 This module is targeted at community pharmacists, in particular those involved in Pharmacy First. It provides guidance on how community pharmacy teams could improve their communication skills to ensure that people understand and can act on the information\u00a0they are given.<\/p>\n\n\n\n<p>This will not just benefit Gypsy\/Travellers, it should benefit everyone who uses the Pharmacy First services.<\/p>\n\n\n\n<p>A link to the module will soon be created on the Pharmacy First website.\u00a0 In the meantime I would be grateful if you could ensure your community pharmacists are aware of the module and encouraged to complete it.&#8221;<\/p>\n<cite>Scottish Public\u00a0Health Network<\/cite><\/blockquote>\n\n\n\n<p><\/p>\n\n\n\n<p>Link: <a href=\"https:\/\/learn.nes.nhs.scot\/32596\/health-literacy\/communication-matters-why-health-literacy-is-important\">https:\/\/learn.nes.nhs.scot\/32596\/health-literacy\/communication-matters-why-health-literacy-is-important<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Resources<\/h2>\n\n\n\n<p>In an effort to improve our patients\u2019 journeys and ensure the most appropriate referrals into the pharmacy first service here are some resources to help -These may be handy to have at reception areas of community pharmacies and general practices with an aim to improve the referral process:<\/p>\n\n\n\n<p><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4921\/general-conditions.pdf\">Table of General Conditions&nbsp;<\/a>\u2013 A short A-Z list of the most common minor conditions that the pharmacy first service is designed to support.<\/p>\n\n\n\n<p><strong>UPDATED!<\/strong>&nbsp;[18\/12\/2023]&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/8437\/conditions-where-a-pgd-exists-for-treatment-nov-23_final-v40.pdf\">Table of National PGD Conditions and Exclusion Criteria Summaries<\/a>&nbsp;\u2013 This short list is an easy reference for&nbsp;<strong>why some people should NOT be referred to community pharmacies<\/strong>&nbsp;for the pharmacy first service, and lists why the national PGDs don\u2019t apply to everyone.<\/p>\n\n\n\n<p><strong>NEW!<\/strong>&nbsp;&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/8434\/20231218-referral_pharmacy-first-pgds-v10.pdf\">This referral form<\/a>&nbsp;has also been produced for use in NHS Fife, at the request of community pharmacy teams and GP staff alike.&nbsp;An e-SBAR is always the preferred and endorsed method of communicating non-urgent clinical information to general practice colleagues &#8211; however this can be used when&nbsp;an e-SBAR has not been possible&nbsp;<\/p>\n\n\n\n<p>NHS Fife Pharmacy Services have&nbsp;produced a set of&nbsp;newsletters for&nbsp;Primary Care teams, particularly aimed at general practice administrative staff.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4931\/practice-update-nov-21.pdf\">Issue 1<\/a>&nbsp;is available now and&nbsp;describes the difference between Pharmacy First and Pharmacy First Plus Service.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4946\/practice-update-nov-21-2.pdf\">Issue 2<\/a>&nbsp;is out now and highlights the most common reasons that people are most likely to be inappropriately referred to community pharmacies \u2013 and where national PGDs do not apply.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4956\/practice-update-nov-21-3.pdf\">Issue 3<\/a>&nbsp;highlights the contribution and impact that community pharmacy teams&nbsp;make through the services they provide and signposts to resources, tools&nbsp;and services that are available to support our patients (e.g&nbsp;<a href=\"https:\/\/www.nhsinform.scot\/scotlands-service-directory\">Scotland&#8217;s&nbsp;National Service Directory<\/a>).&nbsp;This issue also&nbsp;includes details of an interactive&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4957\/practice-update-webinar-nov-21.pdf\">lunch-time webinar<\/a>&nbsp;on 01 and 02 December for GP practice staff.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4971\/practice-update-nov-21-4.pdf\">Issue 4<\/a>&nbsp;mentions what is being done ongoing, ensuring that&nbsp;we continue to work on improving our patients&#8217; journeys in NHS Fife, and how we all can help.<\/p>\n","protected":false},"excerpt":{"rendered":"<p class=\"nhsuk-card__description\">The Minor Ailment Service,&nbsp;delivered by community pharmacies since 2006 has been replaced with the new NHS Pharmacy First Scotland Service.
