Warfarin&nbsp;<\/a>(word document)<\/li>\n\n\n\n<li><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3489\/c64_nmi.doc\">New Medicine Intervention&nbsp;<\/a>(word document)<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">SBAR<\/h2>\n\n\n\n<p>An SBAR is part of the Pharmacy Care Record which is a tool to support communications with your GP Practices and other healthcare professionals.&nbsp;&nbsp;<\/p>\n\n\n\n<p>One of the benefits of using the SBAR tool is that you can send the communication to your local GP practice electronically, reducing the risk of paper copies going missing. &nbsp;&nbsp;If sending by e-mail this&nbsp;<strong>MUST<\/strong>&nbsp;be done through NHS Mail as it contains patient specific details.&nbsp;
