{"id":62,"date":"2024-05-15T11:09:56","date_gmt":"2024-05-15T10:09:56","guid":{"rendered":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-fife\/nhs-borders\/specials\/"},"modified":"2024-05-15T12:43:49","modified_gmt":"2024-05-15T11:43:49","slug":"specials","status":"publish","type":"page","link":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-fife\/pages\/specials\/","title":{"rendered":"Specials"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">Special\/Unlicensed Medicine Procedure NHS Fife<\/h2>\n\n\n\n<div class=\"wp-block-nhsblocks-card1 nhsuk-grid-column-width nhsuk-care-card nhsuk-care-card--type is-style-warning-callout\"><div class=\"nhsuk-care-card__heading-container\"><h3 class=\"nhsuk-care-card__heading\"><span><span class=\"nhsuk-u-visually-hidden\">Non-urgent advice: <\/span><span class=\"nhsuk-care-card__heading-text\">Note<\/span><\/span><\/h3><span class=\"nhsuk-care-card__arrow\" aria-hidden=\"true\"><\/span><\/div><div class=\"nhsuk-care-card__content\">Please note that the email for specials has changed to: <a href=\"mailto:Fife.SpecialsUnlicensedMedicines@nhs.scot\">Fife.SpecialsUnlicensedMedicines@nhs.scot<\/a><\/div><\/div>\n\n\n\n<p>The Scottish Government published&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/1485\/pca2015-p-17.pdf\" target=\"_blank\">PCA(P)(2015)17<\/a>&nbsp;on September 1st 2015 as a national framework for the procurement of Specials\/Unlicensed medicines &#8211; all community pharmacists must be aware of the associated responsibilities, risks and liabilities of procuring and dispensing an unlicensed medicine and ensure the clinical appropriateness of any product supplied.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Every step should be taken to ensure that all licensed preparations and formulations have been fully explored, as well as \u201coff-label\u201d use of licensed products (e.g. opening capsules, dispersing tablets in water etc), before use of a Special\/Unlicensed product is considered.<\/li>\n\n\n\n<li>Specials and Unlicensed products are not required to meet the same standards as licensed preparations.<\/li>\n\n\n\n<li>Pharmacists should\u00a0<strong>always<\/strong>\u00a0contact the prescriber and advise that they have prescribed an unlicensed medicine and to discuss alternatives.<\/li>\n\n\n\n<li>Both prescribers and pharmacists assume a greater responsibility and potential liability where unlicensed medicines are used.\u00a0<\/li>\n<\/ul>\n\n\n\n<p>Once it is certain that an unlicensed medicine is required as there are no other suitable alternatives, local and national NHS Pharmacy Production Units (NHS PPU) should be contacted in the first instance. (Scotland Pharmaceutical &#8220;Specials&#8221; Service: 01382 632 052). Where the item is available from an NHS Pharmacy Production Unit further authorisation is not required.<\/p>\n\n\n\n<p>Where the item is not available from an NHS PPU; Parts 7S and 7U of the Scottish Drug Tariff should be checked for reimbursement arrangements. Part 7S and 7U of the Scottish Drug Tariff can be viewed by downloading&nbsp;<a href=\"http:\/\/www.isdscotland.org\/Health-Topics\/Prescribing-and-Medicines\/Scottish-Drug-Tariff\/Drugs-and-Preparations-with-Tariff-Prices.asp\">Part 7<\/a>&nbsp;(choosing relevant month) and switching tabs at the bottom of the Excel document. If the item is contained in the Scottish Drug Tariff, authorisation is not required.<\/p>\n\n\n\n<p>Where the item is neither contained in Part 7S or 7U of the Scottish Drug Tariff, nor available from an NHS Pharmacy Production Unit, authorisation must be sought from the&nbsp;<strong>prescribing Health Board<\/strong>. For prescriptions issued within NHS Fife, the&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3665\/specials-authorisation-request-from.docx\">Special\/Unlicensed Medicines Authorisation Request From<\/a>&nbsp;&nbsp;should be filled in and sent as an email attachment to&nbsp;<a href=\"mailto:Fife-UHB.SpecialsUnlicensedMedicines@nhs.net\">Fife.SpecialsUnlicensedMedicines@nhs.scot<\/a>&nbsp;this form is also available as an&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3664\/specials-authorisation-request-from.pdf\"><u>Editable PDF<\/u><strong><u>.<\/u><\/strong><\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">\u00a0Please Remember<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The Special\/Unlicensed Medicines Authorisation Request Form must be sent from an\u00a0NHS email address to ensure encryption of confidential patient information<\/li>\n\n\n\n<li>Authorisation is valid for 12 months \u2013 unless otherwise advised.<\/li>\n\n\n\n<li>Reauthorisation is required where the item price increases by more than 20%<\/li>\n\n\n\n<li>Authorisation is given for each patient, not each drug.
