{"id":68,"date":"2024-05-15T11:09:56","date_gmt":"2024-05-15T10:09:56","guid":{"rendered":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-fife\/nhs-borders\/controlled-drugs\/"},"modified":"2024-10-16T14:57:17","modified_gmt":"2024-10-16T13:57:17","slug":"controlled-drugs","status":"publish","type":"page","link":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-fife\/pages\/controlled-drugs\/","title":{"rendered":"Controlled Drugs"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">Reporting Controlled Drug Incidents<\/h2>\n\n\n\n<p>There is a requirement for the  Controlled Drug Accountable Officer (CDAO) to be notified of all incidents and concerns involving controlled drugs (CDs).&nbsp;<\/p>\n\n\n\n<p><span style=\"text-decoration: underline\"><strong>How to report an incident, please see the forms below:<\/strong><\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/forms.office.com\/e\/WhwUyHu0zy\">Electronic Controlled Drug Incident Report Form<\/a> (July 2024)<\/li>\n\n\n\n<li><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/3635\/guide-on-reporting-incidents.pdf\">Guide on Reporting Incidents<\/a><\/li>\n<\/ul>\n\n\n\n<p><span style=\"text-decoration: underline\"><strong>What to report:<\/strong><\/span><\/p>\n\n\n\n<div class=\"wp-block-group is-vertical is-layout-flex wp-container-core-group-is-layout-8cf370e7 wp-block-group-is-layout-flex\">\n<p><strong>1.<\/strong> <strong>Clinical Governance and Professional Practice<\/strong><\/p>\n\n\n\n<p>\u2022 All events or near misses involving prescribing, administration, supply or dispensing of CDs<\/p>\n\n\n\n<p>\u2022 Any concern(s) about professional practice or behaviour of staff in relation to CDs e.g. unusual prescribing patterns<\/p>\n\n\n\n<p>\u2022 Complaints from patients\/carers\/service users relating to CDs<\/p>\n\n\n\n<p><strong>2. Record Keeping and Stock Discrepancies<\/strong><\/p>\n\n\n\n<p>\u2022 Unexplained losses\/discrepancies of any CD, regardless of schedule<\/p>\n\n\n\n<p>\u2022 Any discrepancy in CD stock which, although resolved, raises concerns<\/p>\n\n\n\n<p>\u2022 Events or near misses involving CD destruction<\/p>\n\n\n\n<p>\u2022 Loss of CD Register\/Order Book or other relevant controlled stationery<\/p>\n\n\n\n<p>&nbsp;<strong>3.<\/strong> <strong>Fraud and Possible Criminal Issues<\/strong><\/p>\n\n\n\n<p>\u2022 Any suspected illegal activity relating to CDs, e.g. theft, patients attempting to obtain CDs by deception<\/p>\n\n\n\n<p>\u2022 Lost or stolen prescription forms<\/p>\n\n\n\n<p>\u2022 Attempts to fraudulently produce prescriptions. These examples are not mutually exclusive, for example, record keeping issues may escalate to concerns about clinical practice or suspected theft.<\/p>\n<\/div>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Controlled Drug Destruction \u2013 Requesting Attendance of Authorised Witness<\/h2>\n\n\n\n<p>To request a CD Destruction, please complete the CD Destruction Visit Request form &#8211;&nbsp;<a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/5986\/cd-destruction-visit-request-v3-july-2022.pdf\">here<\/a><\/p>\n\n\n\n<p>In advance of the appointment, Community Pharmacists should:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ensure that all items are recorded in an appropriate CD register<\/li>\n\n\n\n<li>Check that balances are correct<\/li>\n\n\n\n<li>Ensure there are no unresolved discrepancies<\/li>\n\n\n\n<li>Ensure that there are sufficient kits for the volume of CDs requiring destruction<\/li>\n\n\n\n<li>Have a member of staff available for the duration of the appointment<\/li>\n\n\n\n<li>Ensure all staff members have the appropriate PPE<\/li>\n<\/ul>\n\n\n\n<p>Note &#8211;
