These forms can be typed and should be emailed from your clinical mailbox to: ulmrequest@lanarkshire.scot.nhs.uk<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Prescription Endorsement<\/h2>\n\n\n\n<p>Don&#8217;t forget specials\/ULM items should be endorsed with the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>SP (Price of product e.g. if product was \u00a345; endorse SP4500)<\/li>\n\n\n\n<li>HC 3000 (\u00a330 fixed handling fee for Specials\/ULM)<\/li>\n\n\n\n<li>PP (Any additional out of pocket charges e.g. if the product has a \u00a315 delivery fee; endorse PP1500. Prices will be authorised up to 20% over this stated price during this 12 month period. <\/p>\n\n\n\n<p>Clawbacks- The prescribing team will check and clawback any ULM costs that differ from the above.
