{"id":55,"date":"2024-05-15T14:27:36","date_gmt":"2024-05-15T13:27:36","guid":{"rendered":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-lanarkshire\/nhs-borders\/mar-chart-service\/"},"modified":"2025-07-15T14:06:55","modified_gmt":"2025-07-15T13:06:55","slug":"mar-chart-service","status":"publish","type":"page","link":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-lanarkshire\/pages\/pharmacy-services\/mar-chart-service\/","title":{"rendered":"MAR Chart Service"},"content":{"rendered":"\n<p>FAQs (last updated Feb 2024<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What is the MAR Chart Service in Community Pharmacy in NHS Lanarkshire?<\/h2>\n\n\n\n<p>The MAR chart service (Medicines Administration Recording) asks community pharmacies to provide a clinically checked and up-to-date MAR chart for patients whom require medicines administration (Level C) from a Social Care service.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Who is eligible for a MAR Chart in this service?<\/h2>\n\n\n\n<p>Many patients who live at home may require additional support to manage their Activities of Daily Living, with support often provided by family members and other unpaid carers, Social Care services and Health services, or any combination of these.<\/p>\n\n\n\n<p>Some people will require support with their prescribed medication as part of a Social Care package. For these people, an assessment of the level of medication support required will be undertaken by Social Care services in order to identify a level of medication support that matches the person\u2019s needs.<\/p>\n\n\n\n<p>The MAR Chart Service supports patients who have had a social care service assessment, in which this assessment has indicated that these patients require a MAR chart to support medicines administration by a Social Care service as part of their social care package.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How will I know in my pharmacy a patient has had a social care assessment?<\/h2>\n\n\n\n<p>The request for a MAR chart will come from Social Care Services.
