Any patient identifiable information should only be sent electronically to and from an NHS mail account. All cascades should be completed fully &amp; sent to the board.\u00a0 The board <strong>will not undertake any clinical or content checks<\/strong> within the cascades distributed, the reporter is responsible for ensuring the information provided is accurate. Concerns should be verified with the prescriber even if initialled and dated.
