{"id":292,"date":"2024-07-19T11:44:18","date_gmt":"2024-07-31T19:00:19","guid":{"rendered":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-lothian\/pages\/prescriptions-lost-stolen-fraud\/"},"modified":"2026-09-04T09:23:47","modified_gmt":"2026-09-04T08:23:47","slug":"prescriptions-lost-stolen-fraud","status":"publish","type":"page","link":"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-lothian\/pages\/prescriptions-lost-stolen-fraud\/","title":{"rendered":"Prescriptions: Lost\/Stolen\/Fraud"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">NHS National Services Scotland (NSS) has developed guidance to provide NHS Boards in Scotland with a framework for the development of policies, procedures and systems to ensure the security of prescription forms against theft, abuse and fraud.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Security of Prescription Form Guidance can be accessed&nbsp;<a href=\"http:\/\/intranet.lothian.scot.nhs.uk\/Directory\/NonMedicalPrescribing\/Documents\/Security-of-Prescription-Form-Guidance%28Nov%2020%2c%20NHS%20NSS%29.pdf\">here<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Although forgeries are becoming more elaborate, common issues on forged prescriptions may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Unclear or vague prescriber details<\/li>\n\n\n\n<li>Mis-spellings<\/li>\n\n\n\n<li>Unusually high quantities or doses<\/li>\n\n\n\n<li>Out-of-area addresses (of the doctor, service and\/or patient)<\/li>\n\n\n\n<li>Is the prescriber known<\/li>\n\n\n\n<li>Has the title \u2018Dr\u2019 been inserted before the signature<\/li>\n\n\n\n<li>Is the medicine known to be commonly misused<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Prescription Missing\/Lost\/Stolen<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If you have identified a lost\/stolen prescription this must&nbsp;be reported to the&nbsp;<a href=\"mailto:Admin.RehPharmacy@nhslothian.scot.nhs.uk\">loth.adminrehpharmacy@nhs.scot<\/a>&nbsp;by completing a Lost\/Stolen Prescription Form. This enables the healthboard to cascade this information out to the wider Community Pharmacy network to prevent medicines being incorrectly obtained. If the prescription involved a Controlled Drug item, the completed form should also be sent to the Control Drugs Governance Team <a href=\"mailto:ControlledDrugGovernance@nhslothian.scot.nhs.uk\" target=\"_blank\" rel=\"noreferrer noopener\">loth.controlleddruggovernance@nhs.scot<\/a> along with a CDAO form<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-lothian\/wp-content\/uploads\/sites\/14\/DRAFT-Lost-Prescription-Cascade-Template.docx\" data-type=\"attachment\" data-id=\"301\">Lost\/Stolen Prescription Form<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Prescription Pad Missing\/Lost\/Stolen<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you prescribing pads are missing, lost, or stolen&nbsp;as per <a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/media\/4117\/security-of-rx-form-guidance-nov-2020.pdf\">NSS Security of Prescription Form Guidance<\/a> and to support the healthboard to cascade this information, this must&nbsp;be reported to the&nbsp;<a href=\"mailto:Admin.RehPharmacy@nhslothian.scot.nhs.uk\">loth.adminrehpharmacy@nhs.scot<\/a>&nbsp;by completing a Lost\/Stolen Prescription Pad Form and report to the police.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.communitypharmacy.scot.nhs.uk\/nhs-lothian\/wp-content\/uploads\/sites\/14\/DRAFT-Prescription-PAD-Cascade-Template.docx\" data-type=\"attachment\" data-id=\"300\">Lost\/Stolen Prescription <strong>Pad<\/strong> Form<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Fraudulent Prescription<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If you are presented with a prescription that has been amended\/altered, is a photocopy or contains questionable items\/quantities the prescription should be queried with the prescriber. Remember, if there have been any alterations to the script, this should be initialled and dated by the prescriber. Staff also need to be vigilant that alterations have not been added after the prescription has been issued \u2013 all concerns should be verified with the prescriber even if initialled and dated.
