Inverclyde HSCP in conjunction with CPGGC are implementing a new MAR Chart Pilot Service for Level 3/4 patients receiving medication support from HSCP carers. This pilot replaces the existing scheme and current patients will be transferred over in the coming months. Suitable patients will then be identified by the HSCP Team. The anticipated start date for the pilot is 1st July 2025.
The main elements for the Community Pharmacies taking part will be as follows:
- Ordering all prescriptions including for PRN medicines.
- Sychronise medicines and ensure supply is maintained.
- Provide a monthly MAR chart.
- Immediately amend any mid-cycle changes on the patients MAR chart (once notified) and supply medication if required.
- Provide pharmaceutical advice to patient/carer.
- Delivery if required.
The SLA has now been sent to all Inverclyde Community Pharmacies. If you are a branch who is part of a Company that operates a central sign-up process, YOU ARE NOT REQUIRED to sign the SLA as this is co-ordinated from a central point within your Company.
