If you are reporting an incident/concern relating to controlled drugs

The completed form should be forwarded to the CD Governance email address: cdgt@lanarkshire.scot.nhs.uk.
The subject of the email should be – ‘CD Incident Form’.
If you are submitting a request for a CD destruction visit
The CD destruction request form is available on request
Section A of the form should be completed and sent to the CD Governance email address: cdgt@lanarkshire.scot.nhs.uk.
The subject of the email should be – ‘CD Destruction Form along with your pharmacy name’.
Once the returns form is received and approved an Authorised Witness will be in touch to arrange a suitable appointment date.
Pharmacy multiples who have an approved Authorised Witness should contact your local company Authorised Witness to attend to complete a destruction visit and the request should not be sent to the CD Governance email.
Please note: We aim to attend to CD destruction requests within 12 weeks
In advance of the appointment, the person who is responsible for CDs should
Ensure that all items are recorded in an appropriate CD register.
Check that balances are correct 48 hours before appointment.
Ensure there are no unresolved discrepancies.
Have a member of staff available for the duration of the appointment as you/ your staff are required to do the actual destruction; the Authorised Witness attends to witness the destruction only.
Disposal of Patient Returned CDs
‘Patient returned’ CDs are those that have been prescribed for and dispensed to a named patient, and then returned unused or part used for destruction. There is no legal requirement for ‘patient returned’ CDs of any schedule to be destroyed in the presence of an Authorised Witness.
