PGD Authorisations

From 21st September 2026, NHS Orkney will no longer require a copy of individual authorisation forms to be submitted. These must still be signed and retained within the pharmacy and be available for inspection if requested.

Community pharmacists providing NHS services under a PGD must submit the following form to allow NHS Orkney to fulfil its responsibilities to keep a list of all pharmacists providing NHS services under PGDs.
