TransMedD is Translation of Medicines Directions. TransMedD is an easy-to-use written communication form that can help non English speaking patients to understand how to take their medicines. It provides a simple bilingual translation of medicine doses and directions.

TransMedD is available in 12 languages (Arabic, Cantonese, Hungarian, Latvian, Lithuanian, Mandarin, Nepali, Polish, Portuguese, Romanian, Russian and Urdu). Anyone who supplies or prescribes medication can use TransMedD.

What is TransMedD? 

Terms and Conditions for the Use of TransMedD

In order to access the instructions, terms and conditions of use and download TransMedD forms:

  1. Go to NHS Grampian’s Health information Resources website: www.nhsghpcat.org  
  2. In the left hand pane (Browse by Subject), find and click TransMedD
  3. In the right hand pane, select Download. This allows you access to all the TransMedD resources.
  4. For first time users, please print and read the Terms and Conditions and Instructions prior to using TransMedD forms. All users are requested to fill in a registration form.
  5. Select the language you require and print both the TransMedD and the patient feedback form to use (please ask the patient to feedback)
  6. For healthcare professionals, please also print and complete a user feedback form and send to the freepost address provided

We would recommend users to download and print TransMedD forms directly from the website each time you need the form. This will ensure that the most up-to-date version is being used.
