Please note cascade templates should be treated as a security alert and therefore confidential. Any patient identifiable information should only be sent electronically to and from an NHS mail account. All cascades should be completed fully & sent to the board. The board will not undertake any clinical or content checks within the cascades distributed, the reporter is responsible for ensuring the information provided is accurate. Contractors should be vigilant for scripts that have been altered. Concerns should be verified with the prescriber even if initialled and dated. Contractors that are presented with a suspected Fraud Prescription should not dispense the script nor return it to the patient, but report the incident to the police immediately.
Lost/Stolen/Missing Prescription – This template should be used when there has been a written prescription that has gone missing/lost/stolen
Lost/Stolen/Missing BLANK Prescription / Prescription Pad – This template should be used when a blank prescription or prescription pad has gone missing/lost/stolen
Fraud Prescription – This template should be used when an individual has presented with a script & you suspect fraudulent activity
