Local Services (NHS Lothian)

From April 2024, community pharmacy contractors can use the new streamlined annual sign up form process to sign up for the various local services across NHS Lothian where appropriate.

Please find below the Community Pharmacy Local Services agreements for 2024/25 together with a sign-up form.

Process for Signing Up:

The process is as follows:

In the top box of the sign-up form which is for all pharmacies:

Tick every additional service the pharmacy wishes to participate in
Include pharmacist signature beside each service ticked
For Incontinence Urology Products there is one SLA for West Lothian only and a separate SLA for Edinburgh, East and Midlothian – please ensure you tick the correct box.
For Incontinence Urology Products for Edinburgh, East and Midlothian only – the current SLA is extended to 31^st March 2025. In addition to ticking the box on the sign-up sheet, please complete page 7 of the SLA with your details and return signed to communitypharmacy.contract@nhs.scot and continence.care@nhslothian.scot.nhs.uk. Also complete elms2 user access form and return to continence.care@nhslothian.scot.nhs.uk.
For Incontinence Urology Products for West Lothian only – the SLA is being reissued as part of this annual package and runs beyond 31^st March 2025. In addition to ticking the box on the sign-up sheet, please complete and return the sign-up sheet on page 8 of the SLA to Layna.houston@nhslothian.scot.nhs.uk

In the second box of the sign-up form which is for selected pharmacies only:

Tick every additional service the pharmacy wishes to participate in
Include pharmacist signature beside each service ticked
For Palliative Care there is one SLA for ‘in hours’ and a separate SLA for ‘on call’ – please ensure you tick the correct box.

Then:

at the end of the sign-up form print the name of the pharmacist signing the form and include pharmacist signature
Date the form
Return the signed and dated form to communitypharmacy.contract@nhs.scot by Friday 29 March 2024
All return emails must include in the header the PPD code and Signed Contracts Form

Any form that is not signed by a pharmacist will not be processed.

It should be noted that there is no requirement any more to print off individual SLAs for signature and return. All the SLAs have been sent to pharmacies separately and are for information/stored for future reference only.

The completed sign up form that is returned is your signed agreement for all locally negotiated services 2024-25 and is linked to payments, so please review the form and SLAs carefully.
